Enroll in the Senior Adult Independent Living Program
Maricopa County seniors and adults with physical disabilities that want to independently live in their home but need support in reaching their goal may qualify for assistance through the Area Agency on Aging. The Senior and Adult Services Division’s Senior Adult Independent Living (SAIL) program provides support to residents through case management and community referral services.
Please note that services provided through the Area Agency on Aging are dependent on funding and may be available on a short-term basis.
The SAIL program is available to residents throughout Maricopa County. To apply for the SAIL program, follow the step-by-step instructions listed below.
To qualify for program enrollment, you must meet one of the following criteria:
Applicant is 60+ years of age
Applicant is 18 to 59 years of age, has a diagnosed physical disability, and is eligible for/receiving SSI disability payments
Please note that priority is given to individuals with the greatest economic and social need.

By calling the Senior Help Line, you will be connected to an Information & Assistance Specialist who will help you identify the resources and services that will best meet your needs.

Specific to the SAIL program, the Information & Assistance Specialist will determine if you meet the program’s basic eligibility requirements. If you meet the requirements, you may be referred to the SAIL program.
It is important to note that an applicant may be placed on a waitlist for program referral.
Complete In-Home Assessment
Once you have been referred to the SAIL program, a case manager will complete an in-home assessment with you to determine program and service eligibility. The in-home assessment evaluates your ability to perform daily living activities and may take up to 2 hours to complete.
Following the assessment, the case manager will discuss with you SAIL program enrollment and service availability. If enrolled in the SAIL program, goal and service planning will occur between you and your case manager throughout the year.
