  • No registration necessary
  • Parent/Guardian must accompany minor if being vaccinated for consent 
  • Booster doses are available if eligible 
  • Call 602-506-6767 or for Spanish Speaking call 2-1-1 Ext: 271 for assistance 

*Hepatitis A, Hepatitis B, and Tdap vaccine available at no cost for those who are uninsured 

*Pfizer Bivalent Booster (ages 12+) and Moderna Bivalent Booster (ages 18+) are administered at least two months after completion of primary series or two months after the last Monovalent Booster.
